All the things I mentioned above are working together to help me make some positive changes day by day and this has added up over the weeks and months.
But if I had to pick just one thing from the list I'd say having a daily activity plan to refer to has especially helped me. It's a basic template I've made up for a 'regular' day that suggests what I can be doing hour by hour. I'm flexible with it and don't beat myself up if I don't achieve everything on it. Even if I fail it completely I don't beat myself up and just try again the next day. But it gives me something to aim for and has resulted in better daily structure which seems to be improving everything for me.
On top of that, I'd suggest staying around the forums if you find them helpful and posting updates on how you are going. You will get positive feedback here for progress and support at other times.
